In consideration of , a minor not yet having completed his/her 18th birthday (hereinafter “Minor”) being permitted to participate in this mountaineering expedition, I, as the parent or legal guardian of Minor, do hereby give my permission for Minor to participate in same, and I agree individually and on the behalf of Minor to the terms as set forth in the Release of Liability and Assumption of Risk, and do hereby expressly and voluntarily release, forever discharge, and agree to indemnify and hold harmless Expedition-Logistics against any and all claims, demands, causes of action, or liability in connection with Minor’s participation in this mountaineering expedition.
